In short
- Licensed titles: psychologist and psychotherapist require legitimation from Socialstyrelsen.
- Unlicensed title: coach is not protected – anyone can use the word.
- What is actually protected: mainly the title itself, not every conversation that resembles it.
- Register: HOSP, the Register of Authorised Healthcare Professionals, is kept by Socialstyrelsen.
- Not sure which you need? Symptoms that affect daily life belong in healthcare, not coaching.
Psychologist, psychotherapist or coach: what is the difference in Sweden?
A psychologist and a psychotherapist are both licensed healthcare titles in Sweden, issued by Socialstyrelsen. A coach is not licensed at all. Psychologists and psychotherapists can assess, diagnose and treat mental illness; coaches work outside healthcare and cannot.
The confusion is understandable. In many countries "therapist" is used loosely for almost anyone who offers talk-based support, licensed or not. In Sweden the word psykoterapeut means something narrow and legal: a specific licence, held by a specific, checkable person. A coach and a psychotherapist can sound similar in a first conversation, but only one of them is accountable to a healthcare regulator.
Protected titles in Sweden (legitimation)
Legitimation is the Swedish word for a professional licence. Socialstyrelsen issues it for a set of regulated healthcare professions, and psykolog and psykoterapeut are among them. Without a current licence, you may not use those job titles, even if you trained abroad in something similar.
Here is the part worth knowing if you come from a country where the practice itself is licensed, not just the word: Swedish law mainly protects the title, not every activity that resembles therapy. It is illegal to call yourself a psykolog or psykoterapeut without a licence. It is not automatically illegal for an unlicensed person to hold supportive, talk-based sessions for a fee, as long as they never use those protected titles, never diagnose, and never present the work as medical treatment. That is the legal space coaching occupies in Sweden.
What a licensed psychologist does
A legitimerad psykolog (licensed psychologist) has a five-year university degree in psychology, followed by supervised practical training (traditionally known as PTP), before Socialstyrelsen issues the licence. Psychologists carry out psychological assessments, investigations and treatment.
Like other licensed healthcare staff, a psychologist keeps patient records and has a legal duty of confidentiality. That duty does not automatically apply to someone who only calls themselves a coach.
What a licensed psychotherapist does
A legitimerad psykoterapeut (licensed psychotherapist) usually already holds another healthcare qualification – often psychologist, social worker or nurse. They then complete further specialist training in one therapy method, such as cognitive behavioural therapy (CBT) or psychodynamic therapy, before Socialstyrelsen issues the licence.
A psychotherapist delivers structured, licensed treatment aimed at a specific mental health problem, working within the same confidentiality and record-keeping rules as a psychologist.
What a coach does — and does not do
A coach works with people who are basically functioning but want to move forward: habits, decisions, direction, stress at work, a life that changed after a move. A coach cannot diagnose, cannot treat mental illness, and cannot use the words psychologist or psychotherapist, licensed or not, because in Sweden nobody may use them without legitimation.
CoachOnline's own founder is one example of how that line is drawn correctly: he holds a master's degree in psychology (fil. mag.) and is a certified coach, but he is not a licensed psychologist or psychotherapist, and the site never calls him one. That combination is entirely legal in Sweden, provided it is described accurately.
A serious coach also knows when to stop. If what you describe sounds like it belongs in healthcare, a coach should say so and point you towards your vårdcentral rather than keep working with it.
How to check that someone is licensed
Contact Socialstyrelsen directly and ask them to check a named person against HOSP, the Register of Authorised Healthcare Professionals. As a general rule the information in HOSP is public, and Socialstyrelsen uses it to confirm qualifications for pharmacies, employers and the public.
Ask two specific things: whether the licence is current rather than lapsed or withdrawn, and whether the profession on record actually matches the title the person is using. A licence in one regulated profession does not carry over to another.
Which one do you need?
If you have symptoms that affect daily life – low mood, anxiety, sleep problems, trauma that still disturbs you, or a suspected diagnosis – start with your vårdcentral (health centre) or call 1177 for advice. That is also the right first step for something specific like exhaustion disorder, including questions about sick leave and returning to work. See getting mental health help in Sweden for how that system actually works.
If you are coping day to day but stuck on habits, a decision or direction, a coach can be a reasonable place to start instead. Coaching never replaces care you actually need, and a free intro session is a low-stakes way to work out which one fits.
Questions about psychologists, psychotherapists and coaches in Sweden
What is the difference between a psychologist, a psychotherapist and a coach in Sweden?
A psychologist and a psychotherapist are both licensed healthcare titles in Sweden, issued by Socialstyrelsen, the National Board of Health and Welfare. They can assess, diagnose and treat mental illness. A coach is not licensed and cannot do any of that – coaching works with people who are functioning but want to move forward.
What does legitimation mean in Sweden?
Legitimation is the Swedish word for a professional licence. Socialstyrelsen issues it for a set of regulated healthcare professions, including psychologist and psychotherapist. Without it, you may not use those job titles, even if you trained abroad in a similar field.
Which titles are protected by law in Sweden?
Psykolog (psychologist) and psykoterapeut (psychotherapist) are protected titles under Swedish law. Only someone with a current licence from Socialstyrelsen may use them. Coach is not a protected title, so anyone can call themselves a coach regardless of training.
What does a licensed psychologist do?
A legitimerad psykolog has a five-year university degree in psychology plus supervised practical training (traditionally known as PTP) before Socialstyrelsen issues the licence. Psychologists carry out psychological assessments and treatment, and are bound by confidentiality and record-keeping duties like other healthcare staff.
What does a licensed psychotherapist do?
A legitimerad psykoterapeut usually already holds another healthcare qualification, such as psychologist, social worker or nurse. They then complete further specialist training in one therapy method, for example CBT or psychodynamic therapy, before Socialstyrelsen issues the licence.
What can a coach help with, and what can a coach not do?
A coach can help with habits, direction, decisions and stress in someone who is basically functioning. A coach cannot diagnose, cannot treat mental illness, and cannot use the titles psychologist or psychotherapist, because those require a licence a coach does not hold.
How do I check if someone is actually licensed?
Contact Socialstyrelsen and ask them to check the person's name against HOSP, the Register of Authorised Healthcare Professionals. Ask specifically whether the licence is current, not lapsed, and whether it matches the title the person is using.
Do I need a psychologist, a psychotherapist or a coach?
If you have symptoms that affect daily life, such as depression, an anxiety disorder or trauma, start with your vårdcentral (health centre) or call 1177. If you are coping but stuck on habits or direction, a coach may be the better fit.
